To exercise your rights listed in Article 11 of Turkish Personal Data Protection Law No. 6698 (“KVKK”), fill in this form and send it to Yazan Yedek Parça by one of the channels below. You can print the form with your browser’s print command.
How to send it
- In writing: Hand in the signed form in person (showing your ID) or send it by post or through a notary to: Fevziçakmak Mah., Sıla Cad. No: 27, 42050 Karatay / Konya, Türkiye. Please write “KVKK Information Request” on the envelope.
- Electronically: Sign the form with a secure electronic or mobile signature and send it from your registered e-mail (KEP) address, or send it from an e-mail address you have previously given us and that is registered in our system, to [email protected].
We will answer free of charge within 30 days at the latest. If we cannot verify your identity, we may ask for additional information. Requests made on behalf of someone else require a notarised power of attorney or proof of authority.
